Your Opportunity

Mission Australia’s Triple Care Farm is a residential alcohol and other drugs (AOD) program located on 110 acres in the NSW Southern Highlands.
The withdrawal program at Triple Care Farm is a 10 bed residential drug and alcohol program for young people aged between 16-24 years. The detox program is facilitated for a maximum of 28 days and integrates medical support with a psychosocial supports and brief treatment modalities. This program also includes six months aftercare support upon program completion.

You will provide holistic case management support to young people as they participate in the Withdrawal Program and in the community through aftercare support.

Your key responsibilities will be to:
  • Support young people as they enter the program through orientation and induction
  • Support the intake process including the completion of bag searches
  • Complete the weekly update of the list of young people participating in the program
  • Develop a holistic individual case plan that identifies their placement goals
  • Conduct reviews to support participants in achieving their identified case plan goals
  • Liaise with agencies through completing reports and update family/carer network
  • Work to identify treatment and/or accommodation choices for after the program 
  • Create a plan alongside the young person to successfully return to the community
  • Maintain regular contact with young people in the aftercare program
  • Participate in administrative, training and team activities as required

Requirement

  • Certificate IV or equivalent or higher qualification in the field of youth work, Alcohol and Other Drugs, Mental Health or relevant health/human services field
  • Experience in Case management and networking with other agencies
  • Knowledge of mental health , substance use treatment and working with youth
  • Ability to relate to young people with complex needs to make positive changes
  • A current NSW drivers license
  • Evidence of current Covid 19 Vaccinations
